We are seeking a skilled Occupational Therapist to join our team and help deliver expert rehabilitation services to those in need.

Position Overview:
We are looking for an enthusiastic Occupational Therapist to join our Home Health team in providing in-home therapy services to patients recovering from illness, injury, or surgery. In this part-time role (30 hours per week), you will work directly with patients in their homes, helping them regain independence, improve daily functioning, and enhance overall well-being.

Key Responsibilities:

  • Assess patients' physical, cognitive, and emotional abilities to identify areas of need.
  • Develop and implement individualized therapy plans focused on improving the ability to perform daily activities.
  • Provide home safety assessments and recommend modifications to ensure patients' environments are safe and accessible.
  • Collaborate with patients, families, caregivers, and the interdisciplinary Home Health team to provide comprehensive care.
  • Educate patients and their families on proper techniques, adaptive equipment, and self-care strategies to improve daily living.
  • Maintain accurate patient records, document progress, and communicate with the care team to adjust therapy plans as needed.
  • Promote health education and offer resources to support patients' ongoing recovery and well-being.

Qualifications:

  • Master's or Doctoral degree in Occupational Therapy from an accredited program.
  • Current Occupational Therapist (OT) license in the state of Oregon.
  • Experience in Home Health or outpatient therapy preferred, but not required.
  • Knowledge of home safety evaluations and adaptive equipment.
  • Strong interpersonal and communication skills to engage patients and families.
  • Ability to work independently with minimal supervision and collaborate effectively with a multidisciplinary team.
  • Compassionate and patient-centered approach to care.
  • Reliable transportation and ability to travel to patient homes within the service area.
