Pay Range: $52.79-$79.15 an hour depending on experience. Full-time position.
Summary: Responsible for the provision of Pharmacy expertise and oversight throughout the areas of the critical access hospital and the Outpatient Pharmacy at Spanish Peaks Regional Health Center, where drugs and biologicals are administered and stored.
Essential Job Functions
Develops, maintains and oversee an effective Pharmaceutical and Therapeutics program.
Oversees and participates in inventory activities (monitors stock levels).
Maintains and updates as necessary current price lists.
Places orders for drugs.
Checks in narcotics received.
Oversees repackaged/repackaging drugs/records.
Identifies, quarantines and replaces outdated unusable medications.
Participates in Infection Control, Quality Improvement and other hospital activities as required.
Ensures compliance of drug storage and drug preparation areas within the pharmacy and throughout the facility.
Develops and enforces policies and procedures relating to the safe and effective use of drugs in the facility.
Conducts regular evaluations with all clinical staff members regarding the need for education programs.
Provides clinical education as determined.
Ensures the pharmacy areas are kept clean, neat and well organized.
Interprets medication orders.
Maintains patient profiles.
Monitors patient drug therapy regimens for contraindications, interactions, allergies and appropriateness.
Reconciles patient profiles and medication administration records.
Maintains controlled substance records as required by law and pharmacy policies and procedures.
Obtains and verifies medication histories according to policy.
Provides drug information to hospital professional staff members and patients.
Available to provide counseling to patients on drugs to be taken after discharge.
Coordinates return of outdated and unusable drugs via pharmaceutical return services.
Monitors adverse drug reactions.
Evaluates medication orders and contacts physician(s) if needed for clarification, correction, etc.
Periodically evaluates existing dosing protocols, Aminoglycoside and Vancomycin evaluation of all orders and dosing as required. Completes Anticoagulant monitoring.
Manages formulary.
Manages Pharmacy Information System.
Oversight of facility Pyxis system.
Serve as a preceptor for pharmacy students and residents.
Continually contribute to the success of SPHRC through support of the strategic plan pillars, which are service, quality, finance, people and growth.
Ensure the consideration of the role of cultural, social and behavioral factors in the accessibility, availability, acceptability and delivery of pharmacy information and services.
Ensure all required licensing, certification and/or education requirements for assigned staff members are completed and up-to-date.
Perform other functions as required to fulfill the expectations of the Pharmacist.
Requirements:Minimum Required Education/Experience: Graduation from an accredited College of Pharmacy with a Bachelor's or PharmD degree. At least two years of clinical patient care and orders management experience in a hospital setting.
Pre-Employment Knowledge, Abilities and Skills:
Knowledge and skills related to computerized pharmacy systems, special prescription delivery systems, third party payor and fiscal reimbursement knowledge related to ambulatory care practices.
Outstanding verbal and written communication skills.
Knowledge of financial management; knowledge of practices and procedures for planning and formulating budgets and monitoring related expenses.
Knowledge of principles and practices of retail pharmacy operations.
Ability to establish and maintain effective relationships with the public, other agencies, employees and administration.
Ability to react to frequent changes in duties and volume of work.
Note: An acceptable combination of education, training and experience that provides the above knowledge, abilities and skills may be substituted.
Necessary Special Requirements: Must obtain annual flu and Covid vaccination and any job-specific immunizations. Required to submit to a tuberculosis screening process.
Licenses or Certificates: Active Colorado pharmacist license.
Unusual Demands: Work is subject to recurring and inflexible deadlines and frequent interruptions.
