Physical Therapist Job DescriptionWe are looking for a motivated Physical Therapist to join our integrated medical practice. The ideal candidate is passionate about working closely with patients, creating long term relationships and providing high-quality, individualized care.About us:We are an integrative physical medical clinic serving patients in the Fort Wayne area. We offer innovative non-surgical options for those suffering from pain conditions.Duties:Review patient health and medical histories, examine patients and review pertinent medical records.Consult with referring physicians and healthcare team regarding diagnosis as needed.Develop and implement physical therapy treatment programs based on each patient's needs.Assist patients with all phases of physical therapy treatment and techniques.Patient education on diagnosis, treatment plan options and lifestyle modifications.Documentation and charting of patient information.Work cooperatively with the multidisciplinary team for comprehensive patient care.Patient follow up to ensure treatment goals are being met.Requirements:Graduation from an accredited Physical Therapy Program.Active Physical Therapy license in IN.Work Schedule:Part time to Full time.Compensation:$42-$50/hour.Benefits:2 weeks' vacation/PTO.Professional growth and mentorship opportunities.No Weekend work.
