Job Description
The job seeks a Full Time Home Health Registered Nurse (RN) Care Manager to coordinate and direct patient care. The nurse evaluates patient needs, develops a care plan, educates patients and families, and monitors patient response to achieve goals.Responsibilities include: Evaluating patient needs and developing a care plan in conjunction with the physician Educating patients, families, caregivers, and community providers to safely perform care Providing follow-up by evaluating the effectiveness of the care plan and monitoring patient/family's responseThe ideal candidate will have: A minimum of one-year relevant clinical experience A current Registered Nursing License in Florida A valid Driver's License and current car insurance BLS CertificationThis position requires a Full-time schedule with rotating weekends and some call.Location: This position will primarily do visits in the greater Downtown Orlando area.AdventHealth is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
