Required SkillsREPORTS TO: Clinical Nurse Manager / CoordinatorKey Responsibilities:1. Provides direct care for patients in the clinic setting including holistic assessment,comprehension2. Evaluates wounds according to protocol, and appropriately prepares the patient medically .3. Integrates diagnostic data into health assessment notifying physicians of results in a timelyfashion and documenting appropriately,4. Uses knowledge and understanding of algorithms for wound care.5. Exercises clinical competence in nursing skills and follows procedures based on WCC, andhospital based policies and procedures.6. Administers pharmaceutical as ordered.7. Demonstrates competence in ordered treatments including but not limited to vital signs,capillary blood sugars, vascular assessments, wound assessments, sterile technique, dressings,and sterile syringe fill techniques.8. Demonstrates competence in-patient and family education on common topics including but not limited to wound care, dressing applications, compression therapy, diabetic foot care, andvenous life style changes and wound off-loading.9. Adheres to guidelines or standards of practice (i.e. ANA), community, associate hospital, AMD,all current WCC policies and procedures.10. Safety precautions are implemented as needed by professional judgment/policy andprocedures.11. Demonstrates consistent use of OSHA guidelines and Universal Precautions.12. Communicable diseases are reported per policy.13. Utilizes proper body mechanics.14. Reviews and completes patient history, holistic nursing assessment and evaluation on all new patients.15. Demonstrates competent utilization of interview, examination and observation techniques withpatients and significant others.16. Prepares and implements a plan for care of wound patient by second outpatient encounter usingestablished algorithm, policies and procedures in collaboration with the physician, the patientand significant others.17. Keeps patient plans for care updated to reflect changes in conditions and response to therapiesor education.18. Implements and evaluates plans for care reflecting appropriate nursing and medical diagnoses,goal development, outcome criteria and target dates.19. Assumes patient advocate role protecting all patient rights.20. Provides continuity of care for patients as plan of care may involve multiple disciplines, products,and sites for care including inpatient care, outpatient procedures and clinic care.21. Utilizes available resources effectively by appropriate consultations and collaboration tomaximize effectiveness of care.22. Consistently responds to internal and external customers in a professional manner.23. Supports and encourages a harmonious work relationship assuming additional responsibility andassisting others without being asked.24. Demonstrate flexibility with staffing needs coordinating with physicians, patients and co-workers.25. Delegates tasks to appropriate skill level providing needed supervision for quality followthrough.26. Prioritizes to consistently meet deadlines and keep documentation current.27. Participates in the CQI process, review and revision of policies and procedures.28. Handles daily and routine problems following proper channels of communication and recognizescomplex issues that need referral with recommendations to management.29. Assumes active role in WCC by participating at staff meetings and other center related activities;30. Reviews minutes when unable to attend.31. Attends all mandatory education programs and some additional continuing professionaleducation functions to enhance skills and judgment.32. Communicates to Nurse Manager or Program Director areas of educational or proceduraldeficiency.33. Reviews current literature sharing ideas for care of patients and revision of policies andprocedures.34. Consistently initiates and actively participates in implementing change to improve workenvironment, work processes or patient care.35. Documents accurately and promptly patient assessment, care and response to treatment andeducation on approved forms.36. Documents accurately reflect changes in patient status and appropriate actions.37. Documents clearly and legibly using only approved abbreviations.38. Actively keeps patient informed of anticipated responses to treatment.39. Facilitates discussion between patient and physician.40. Seeks financial counseling regarding reimbursement or other medical financial concerns ofpatient or significant other.41. Ensures all necessary informed consents are signed prior to providing non-emergency care.42. Responds accurately and timely to patient financial questions either by knowledge of answer orwith referral source and follow-through.43. Documents consumption of products and charges appropriately.

Minimum Work ExperienceAt Least 1 Year Wound Care ExperienceBLS and ACLS Required within 3 months of hire. PALS Preferred but not required.CWON, WOCN, CWS, WCC, CHRT, or CHT preferred. Selectee will be required to be either enrolled in a Wound Care Specific Credential Program or a Wound Care Specific Continuing Education Program whether it be to become Credentialed in Wound Care or perform a minimum of 2 Wound Care Continuing Education Hours Per Month.
